A tabular overview of the recommended vaccination schedule (basic immunization and booster vaccination) after SARS-CoV-2 infection in immunocompromised individuals can be found in Table 7 of the 18th update of the STIKO recommendation.
STIKO recommends administration of one vaccine dose as part of basic immunization for unvaccinated immunocompromised individuals 12 years of age and older and for 5- to 11-year-old children with preexisting conditions who have undergone confirmed SARS-CoV-2 infection*. If infection was confirmed by PCR test, vaccination should be given at least 3 months after illness.
If the infection was confirmed by serological detection of specific antibodies in a blood sample, vaccination can be given as early as 4 weeks after laboratory diagnosis. In persons with immunodeficiency, it must be decided on a case-by-case basis whether a single dose of vaccine is sufficient or whether a complete vaccination series should be administered. Children without preexisting conditions who have already experienced laboratory-diagnosed SARS-CoV-2 infection should not be vaccinated for the time being.
Individuals who have had confirmed SARS-CoV-2 infection less than 4 weeks apart from the previous vaccination after the 1st vaccine dose will receive a 2nd vaccine dose with an interval of 3 months or more from the infection as part of basic immunization. If serologic evidence of SARS-CoV-2 infection is present, administration of a vaccine dose may also be given as early as 4 weeks apart from laboratory diagnosis. If the SARS-CoV-2 infection has occurred at an interval of 4 or more weeks from the previous 1-time vaccination, no further vaccination is necessary for basic immunization.
For the First Booster Vaccination:
- Persons who have experienced SARS-CoV-2 infection and received a dose of vaccine at least 4 weeks apart thereafter to enhance immune protection should receive a booster vaccination at an interval of at least 3 months after the previous vaccination.
- Individuals who have undergone SARS-CoV-2 infection after COVID-19 vaccination (regardless of the number of vaccine doses for basic immunization) should also receive booster vaccination at an interval of at least 3 months after infection according to the STIKO recommendation.
- However, if the SARS-CoV-2 infection occurs at an interval of at least 3 months after the previous vaccine dose and the basic immunization consisted of 2 vaccine doses, no booster vaccination is necessary until further notice.
For the time being, a second booster vaccination is not recommended for individuals who have experienced SARS-CoV-2 infection after receiving the first booster vaccination.
* Detection of confirmed, passed-through SARS-CoV-2 infection can be done by direct pathogen detection (PCR) at the time of infection or by detection of specific antibodies that prove passed-through infection. The laboratory diagnostic findings should have been obtained in a laboratory working in accordance with the guidelines of the German Medical Association for quality assurance of laboratory medical examinations (RiLiBÄK) or accredited in accordance with DIN EN ISO 15189.
